A teacher attendance certificate is an official document confirming that a school teacher attended and performed examination duty at a designated examination centre on specified dates. It records the teacher, working school, examination college, duty dates, and appointed role — and is required for duty allowance claims and official records.
This version is tailored for school teachers deputed to examination duty. It captures the teacher's working school name and address (free text) rather than a working college code, and offers flexible duty-date entry — either a continuous From-To range or multiple individual dates — which suits teachers whose duty falls on scattered days.
From & To Dates is for a continuous duty period — pick a start and end date and all days in between are covered. Multi Dates lets you pick several separate, non-continuous duty days from a calendar. Choose the format that matches how the teacher's examination duty was scheduled.
After choosing Multi Dates in the date format dropdown, click the date field to open a calendar and click each duty date you want to include. Selected dates are collected together and saved with the certificate. Click a selected date again to deselect it.
Type the Examination College Code — after 4 characters an autocomplete list of matching colleges appears. When you leave (blur) the field, the code is validated against the college database and the matched college name is shown in green, or an error in red if the code is not found.
Enter the full name and address of the school where the teacher is permanently posted. This is a free-text field (converted to uppercase) and appears on the certificate to identify the teacher's home institution, distinct from the examination centre where duty was performed.
The available roles are: Examiner, Chief Superintendent, Departmental Officer, Assistant Examiner, Practical Examiner, Chief Examiner, ACO-I, ACO, Scrutinizer, Invigilator, Custodian I, and Custodian II. Invigilator is pre-selected by default as it is the most common teacher duty — change it to match the official appointment.
The date format determines which date fields are shown and validated. Until you select From & To Dates or Multi Dates, no date fields appear, so choosing a format first ensures the correct duty dates are captured on the certificate. The form will not have valid dates if this step is skipped.
Yes. Submitted certificates can be retrieved from the certificate records list, edited, and re-submitted. Open the relevant record, make your corrections, and re-submit to update the certificate document.
